By electronically signing and submitting this application you agree to be bound by the following terms and conditions. Furthermore, your electronic signature has the same validity and effect as if you had signed your signature on paper by hand. If you do not agree please do not submit this application. CEG Life Insurance Services will not be held liable for how your information is handled by any of these third-party representatives. Any such concern related to how these parties handle your information must be addressed to that specific party. Additionally, though we make every effort to preserve the privacy of your information, we may need to disclose personal information when required by law wherein we have a good-faith belief that such action is necessary to comply with a current judicial proceeding, a court order or legal process served on our website.

No Cost to Apply
There is no cost to you for any part of the application process. You will only have to pay the premium for your life insurance policy, if you are given an offer and choose to accept it.
No Obligation to Purchase
By submitting an application you authorize us to initiate a company-specific application on your behalf. The company-specific application will then be signed by you: either electronically through a secure online signature process, over the phone with an insurance company representative, or by hand upon receipt of the application. Additionally, by signing the company-specific application you allow the insurance company to perform an underwriting analysis of your health history, as well as your current health and lifestyle. After this is completed, if a company gives you an offer of insurance you are free to accept, reject, or modify it.
How We are Compensated
If you choose to accept and pay for any insurance policy that you apply for through our agency or one of our agents, we will be paid directly by the insurance company that issues your policy. There is no extra charge for working with us to obtain life insurance coverage, nor to have us service your policy needs in the future.
Scope of Financial Planning Engagement
All financial planning services initiated through our website are limited to assisting you in obtaining the insurance coverage requested, regularly monitoring and reviewing any policy purchased through our agency, and assisting with future service requests, unless otherwise stated or arranged.
